Please print these documents for your first visit if you are a new patient.

Core Elements is an Out-of-Network Provider. Payment is due at the time of service. I will provide you with a "super-bill" that you can submit to your insurance company for out-of-network benefits. Out-of-network benefits are determined by your individual insurance plan.
